Inspector Notes
An unannounced monitoring inspection was conducted on 01/01/2023 from 8:00am to 3:00pm. There were 75 children ages 3 years old to age 5 years old supervised by 10 staff. The physical plant, programming, attendance, menus, 5 staff records, 5 children?s records, 2 medications with corresponding authorization records, emergency drills, emergency supplies, and policies were reviewed. Due to the rain, the playground was not inspected. Children were observed participating in reading, math activities, and outside play. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

4Evidence:
1)The record for Staff #1 (date of hire 1/23/23) did not contain documentation of a completed criminal history record check or central registry check from all states where they resided within the last five years prior to employment.
2)The record for Staff #2 (date of hire 11/7/22) did not contain documentation of a completed central registry record check from all states where they resided within the last five years prior to employment.

Evidence:
1)In the Full day, 5 day 4s classroom, there was a bleach mixture spray and a disinfectant spray in the classroom in an area that was unlocked.
2)In the restroom area for the center, there were antibacterial wipes on a table in an unlocked area.
3)In the unlocked staff room across from the children?s restrooms, there were 20 containers of antibacterial wipes on a shelf that was not locked.
All chemicals are properly stored and locked.
Evidence: For medications for Child #5, the duration of the parent?s authorization was not renewed after 10 workdays nor was written medication authorization given from the child?s physician and parent for a long term medication.
Evidence:
1)In the Full day, 5 day 3s classroom, there was an Epipen for Child #1 that was stored on an open shelf that was not locked.
2)In the Monday through Friday, 5 day 4s classroom, there was an Epipen for Child #5 that was stored in a tote bag.
