Inspector Notes

An unannounced renewal inspection was conducted on 07/19/2023 from 10:30am to 2:30pm. There were 84 children ages 3 years old to age 12 years old supervised by 13 staff. The physical plant, outdoor playground areas, programming, attendance, menus, 5 staff records, 5 children?s records, 2 medications with corresponding authorization records, emergency drills, and policies were reviewed. Children were observed participating in art, lunch, and outside play. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT) and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Based on documentation review, the center did not have at least one staff member in each classroom or area where children are present that had obtained first aid and cardiopulmonary resuscitation (CPR) certification.

Evidence: Four total classrooms (Two of the 3s classrooms, the 1st & 2nd grade classroom, and the 3rd and 4th grade classroom) did not have at least one staff member that had documentation of First Aid and cardiopulmonary resuscitation (CPR) certification.
Plan of Correction: Hazelwild will get all employees certified in First Aid and CPR in a timely manner.
