Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on August 19, 2021 and concluded on September 7, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 80 children present directly supervised by 20 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. The children were observed eating lunch. The inspector reviewed 7 children records and 7 staff records.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on children records review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: Child #3 did not have documentation of immunizations in the record available for review during the inspection.
Plan of Correction: We will require all students (even summer guest to provide documentation before starting)

Based on children records review, the center did not obtain documentation that each child has a physical examination before the child's attendance or within one month after attendance.
Evidence: Child #3 did not have a physical examination in the record to review during the inspection.

Standard 22VAC40-185-210-A
Based on staff records review, the center did not obtain documentation of program leaders have fulfilled a high school program completion or the equivalent.
Evidence:
1. Staff member #2 (date of employment 6/11/1994) did not have documentation of high school completion or the equivalent in record available for review.
2. Staff member #3 (date of employment 9/11/2017) did not have documentation documentation of high school completion or the equivalent in record available for review. Staff member's Child Development Associate (CDA) expired on 4/22/2021.
Plan of Correction: Both staff members had their CDAs which expired. They are both working on them renewed.

Based on staff records review, the center did not obtain documentation that two or more references as to character and reputation as well as competency were checked before employment.
Evidence: Staff #1 (date of employment 4/29/2021) did not have documentation of two or more references before employment.
