Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 2/5/2026 and completed on 2/5/2026, as a part of the licensure period.The on-site inspection began at 11:22am and ended at 1:12pm. The inspector reviewed compliance in the areas listed above. There were 63 children present and 19 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

1The record for Staff #4 (employed for six years and four months) did not contain documentation of the education required by the job position.
