Standard 8VAC20-780-510-B
Prescription medication shall be given to a child:
According to the center's written medication policies; and
Only with written authorization from the parent.

The medication authorization forms for two prescription medications for Child 1 had expired over 30 days ago. The medications were administered more than ten times during the expired period. The center's policy states "staff is not authorized to administer any prescription medication without written authorization from a parent and physician".
Plan of Correction: Going forward we will audit records for expiration dates on forms. No medication will be given without proper authorization.

When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed.

Two medications for Child 1 were onsite, but the medication authorizations had expired over 30 days ago. The authorizations had not been renewed and the medications were not returned or disposed of as required.
Plan of Correction: Authorizations will be updated or medication will be given back to parents.
