Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 05/17/2022, between the hours of 10 am to 11:30 am. There were 72 children present, ranging from infant to preschool-age, in 9 separate rooms, with a total of 21 staff supervising. The inspector reviewed areas of background checks; physical plant; staffing and supervision; medication; and emergencies. A total of 16 staff records were reviewed.

Information gathered during the inspection determined that there was (1) one non-compliance with an applicable code. This violation was documented on the Violation Notice issued to the program.

Based on review 1 of 16 staff files indicated that the facility has expired background documentation that is older than 5 years.

Evidence -
1. On the date of inspection (05/17/2022), the Search of the Central Registry (CPS) check for Staff #9 was dated - 11/08/2016.
2. The Sworn Statement or Affirmation form (SDS) for Staff #9 was dated - 04/28/2017.
Plan of Correction: Both an updated CPS check and an updated SDS will be completed. The CPS check will be sent to obtain findings. All results will be shared with Licensing Inspector for his review.
