Inspector Notes

An in-person, annual code compliance inspection was initiated and concluded by a representative from the VDOE, Office of Child Care Health and Safety (Fairfax) on 05/19/2023, between the hours of 9:30 am through 12:30 pm. There were 87 children present, ranging in infant to preschool-age, 10 months to 5 years, with 19 staff supervising, across a total of 9 classrooms. The inspector reviewed compliance in the following areas: posting of the written disclosure statement, qualifications, physical plant, capacity, food services, health reports, ratio and staffing, supervision, handwashing, daily health screenings, staff trainings, and background checks. The Inspector reviewed (13) staff records to ensure that the documentation was complete and in compliance.

Information gathered during the inspection determined (2) areas of non-compliance with applicable code sections, and the violations were documented on the violation notice issued to the facility.

Based on review, 1 of 9 children files indicated that the facility did not ensure that all children in the center were in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases.

Evidence -
1. On the date of inspection (05/19/2023), an immunization page was not available for Child #7.
2. The start date for Child #7 was listed as 09/06/2022.
Plan of Correction: The parent will be contacted and this immunization page will be placed on file.

Based on review, 1 of 13 staff files indicated that not all fingerprint check (FPC) have been completed within the past five (5) years.

Evidence -
1. On the date of inspection (05/19/2023), a FPC for Staff #11 was dated - 03/14/2018.
2. The director confirmed that this was the most current FPC on file for this staff person.
Plan of Correction: An appointment to have this FPC updated will be scheduled within the next 10 days. The finding (once obtained) will be placed on file.
