Inspector Notes
The Licensing Inspector initiated and completed an unannounced monitoring inspection on 5/10/2023. The inspector was on site from 10:15am until 12:45pm.
There were 54 children under the care of 10 staff. The ages of the children ranged from twenty-two months to four years old. The children were observed during indoor free play, outdoor play, lunch, and naptime. Five children?s record and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

2Evidence: Staff #5, hire date of 8/15/2022 had fingerprint eligibility letter was dated 8/26/2022.
Evidence: Staff #2, which was hired on 7/7/2022 had record of a TB test/ screening dated for 7/18/2022.
Staff #4, which was hired on 7/19/2021 had record of a TB test/ screening dated for 10/26/2021.
