An unannounced monitoring inspection was initiated and completed on 07/10/2025 in response to a self-report received by the licensing office on 07/03/2025 relating to an injury of a child requiring outside medical treatment. The inspector reviewed 1 child?s records and 1 staff records on site on 07/10/2025. This inspection included, document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat violation
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child #1, who had been attending for over a year, had documentation of an immunization record on file that were dated over a year after child?s first day.

There was a maximum of 2 inches of resilient surfacing under the 7 foot playground equipment. 9 inches of resilient surfacing would be needed.
