Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced monitoring inspection was initiated on 04/16/2021 and concluded in 04/19/2021 in response to a self-report incident that occurred on 04/15/2021. Interviews were conducted with staff, documents were reviewed and staff/child records were reviewed.

Based on record review and interview, it was determined that the center did not ensure that Central Registry search results are obtained every five years for employees.
Evidence:
1) The most recent Central Registry search request results available in the record for Staff #1 were dated 04/04/2016. The director confirmed this is the most recent Central Registry search results in Staff #1's record.
Plan of Correction: A new Central Registry search was requested on this date for Staff #1.
