An unannounced complaint inspection was conducted on 12/14/2021 to investigate a complaint received by the department on the area of supervision and ratios.
On this date, observations were made, interviews were conducted with staff, and documents were reviewed.
The information gathered during the inspection did not support the allegations of the complaint so the complaint is determined to be not-valid. Areas of non-compliance not related to the complaint are listed in the Violation Notice. Based on documentation review and interview with staff, the center did not maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.
Evidence:
1) On the date of inspection, an accurate written record of children's injuries was not available for review. The inspector requested the "injury report log" to review and the facility representative was able to locate only two reports. (12/01/2021 and 12/06/2021)
