Inspector Notes
An unannounced complaint investigation was conducted on 04/06/2022 from approximately 11:20 am to 1:45 pm in response to a complaint received by the licensing office on 03/29/2022, regarding staff to children ratios, building maintenance and emergency procedures.
Observations were made in the center, records and policies were reviewed, and interviews with staff were conducted. Based on the information gathered during the investigation, the preponderance of evidence supports the allegations in regards to emergency procedures; therefore, the complaint was determined to be partially valid. The complaint related violations are identified on the Violation Notice. Violations
2Evidence:
1) In a telephone interview, Adult #1 stated that on 03/15/2022 afternoon, they noticed a bump on Child #1's head. Child #1 stated that they had tripped earlier in the classroom and bumped their head. Adult #1 stated that they were not informed of the incident when they went to pick up Child #1. Adult #1 went to the center the next morning to inquire about the incident and was told by Staff #1 that on 03/15/2022, Child #1 had reported that they fell and bumped their head.
2) In an interview with Staff #1, they stated that on 03/15/2022, Child #1 came and said that they had bumped their head. Staff #1 stated that they "briefly looked at it" and they did not notice a bump on Child #1 's head and that Child #1 was not crying or complaining.
Evidence:
1) During a telephonic interview, Adult #1 stated that on 03/15/2022, Child #1 told them that they fell and bumped their head earlier on that day. Adult #1 stated that, at pickup time, they were not informed of the incident and did not receive an injury report.
3) During an interview on the date of inspection, Staff #1 stated that on 03/15/2022, Child #1 came and said that they had bumped their head. Staff #1 stated that they did not fill out an incident report that day (03/15/2022); that they completed an incident report the next day when Adult #1 requested it.
