During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced monitoring inspection was initiated on October 16, 2025 and completed on October 30, 2025 in response to a self-report received by the licensing office on October 10, 2025 relating to supervision. The inspector reviewed 1 child?s record and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on November 6, 2025. A POC submitted after this date will not appear on the public website.
Violations
3Child #1, (2 years old), was left alone for approximately 6 minutes on the playground while the rest of the class went inside. The staff did not realize the child was missing until their parent came for pick up.
Child #1, (2 years old), was left alone for approximately 6 minutes on the playground out of sight and sound supervision of staff while the rest of the class went inside. When staff went to look for the child, Child #1 was found inside of an empty classroom that they had entered through an open door from the playground.
The center's supervision policy requires when children are moving from one place to another, the ?Child Supervision Record (CSR)? is to be used for transitions and to avoid a child being left unattended. On October 9, 2025, Child #1 (2 years old) was let on the playground unsupervised for approximately 6 minutes. Staff #1, Staff #2, and Staff #3 stated they did not complete the CSR when coming in from the playground.
