Inspector Notes

Today an unannounced renewal inspection took place between the hours of 11:30am-1:05pm. There were 5 classrooms observed with a total of 82 children with 15 staff. The children were observed eating lunch, having tummy time, diapering, hand washing, laying down for their designated rest period and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, medication and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Based on a review of staff records, 1 out of 4 staff records reviewed did not have a current sworn statement or affirmation on file.
Evidence:
1. Staff B (doh: 1/13/2003) had a sworn statement on file that was dated 12/8/2017.
Plan of Correction: I will have the employee sign a new sworn statement today.

Based on observation, the center did not maintain the required staff to child ratios in the Pre-K classroom.

Evidence:
1. At approximately, 12:01pm, Staff A was observed to be in the Pre-K classroom supervising 12 children. Staff A stated "The other teacher (Staff B) is in the bathroom with the other half of the children." At approximately, 12:06pm, the Director stepped into the classroom to support Staff A in maintain the required ratio of 1 staff member for every 10 children until Staff B returned from the bathroom area, which is located down the hallway.
Plan of Correction: The ratio issue was corrected during the inspection by the Director stepping into the classroom.
