Inspector Notes
A Subsidy Health and Safety inspection was conducted on 03/04/2024 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

1) Staff # 2 did not have documentation of taking the department's health and safety update course. Last training certificate was not available for review.
2) Staff # 1 did not have documentation of taking the department's health and safety update course. Last documented training update was 03/29/2022.
3) Staff # 3 did not have documentation of taking the department's health and safety update course. Last documented training update was 11/02/2021.
4) Staff # 4 did not have documentation of taking the department's health and safety update course. Last documented training update was 03/09/2022.
