Inspector Notes
A complaint was received by the licensing office on May 29, 2024, in regard to special feeding needs and staff qualifications and training. An investigation was initiated on May 31, 2024 between the hours of 10:25am - 1:05pm, and concluded on June 5, 2024. Observations were made, interviews were completed, and documentation was reviewed. The preponderance of evidence gathered supports violations of the standards. Areas of noncompliance can be found in the violation notice. Areas of noncompliance not related to the complaint but identified during the course of the investigation can also be found on the violation notice. Violations
51. Staff #1's (date of hire: 01/26/09) and Staff #4's (date of hire: 11/25/19) record's did not contain documentation of the prevention of shaken baby syndrome and abusive head trauma including procedures to cope with crying babies and distraught children.

Evidence:
1. In the Infant A classroom, 3 crib mattresses were cracked, exposing the foam on the inside.
Evidence:
1. Staff #2 placed Child #1 (age: 8 months) in their crib on their stomach.
2. Staff #2 acknowledged that they placed Child #1 in their crib on their stomach.
Evidence:
1. The menu posted outside of the kitchen was not dated for the current one-week period.
2. In the Infant B classroom, child #1's (age: 8 months) menu was not dated for the current one-week period.
Evidence:
1. Child #1's (age: 8 months) parent provided instructions indicates that child #1 should be fed at approximately 7:30am, 10:30am, 1:30pm and 4:30pm.
2. Child #1 arrived on 05/28/2024 at approximately 9:06am. Child #1 took a nap from approximately 10:02am-12:12pm. Child #1 was fed their first bottle at approximately 1:15pm.
