Inspector Notes

An unannounced monitoring inspection was initiated on June 26, 2025, and completed on July 8, 2025, in response to a self-report received by the licensing office on June 24, 2025, relating to Supervision of children. The inspector reviewed 1 child record and 3 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 15, 2025. A POC submitted after this date will not appear on the public website.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On June 23, 2025, child #1 (age: 2 years), was out of sight and sound supervision for an undetermined amount of time. The child was found in an area outdoors, enclosed between the child's classroom #1 and the playground. The child was found in a shaded area and had removed their shoes from their feet.
Plan of Correction: Staff retrained on active supervision, CSR's and mandated reporting.
