During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on August 25, 2025. The on-site inspection began at 9:15am and ended at 11:20am. The inspector reviewed compliance in the areas listed above. There were 58 children present and 11 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 1, 2025. A POC submitted after this date will not appear on the public website.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

In classroom #1, the diaper changing table was observed dirty with accumulated debris on the side edge between the non-absorbent surface and side of the table.

On the playground, there were two foot pedals missing from one bicycle, the small garden fence was broken, exposing sharp plastic pieces and the plastic playhouse had a small hole in it, exposing sharp plastic.
Plan of Correction: Utilize daily cleaning checklist and proper table cleaning. Bicycle was thrown away, fence was thrown away and playhouse hole is covered.

If diaper ointment or cream is used, these products do not need to be kept locked but shall be inaccessible to children.

In classroom #1, diaper ointment was observed in an unlocked cabinet under the changing table, accessible to children.
Plan of Correction: Replace locks on cabinet and move diaper creams to higher storage.
