Inspector Notes
An unannounced monitoring inspection was conducted on09/25/2025 as a part of the licensure period. The on-site inspection began at 10:00am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements.There were 79 children present and 15 staff. The inspector reviewed 6 children?s records and 2 staff record. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
2The bathroom in the 3 year-old room had peeling paint on the wall and missing tiles at the base of the wall with jagged edges. Some of the other tiles were loose.
In the Discovery Preschool room the windowsill's black paint was peeling and chipped, the plastic edging on one of the toy shelves and teacher cabinet were broken in areas and has sharp edges and there was peeling paint on the door fames. the bathroom in this same classroom had an open box of rubber gloves and a stack of plastic bags on the floor, in reach of children.
Both bathrooms in the Pre-Kindegarten room had cracks in the floor tile with debris and rust, where screws were used.
minimum safety standards when tested in accordance with the procedures
described in the American Society for Testing and Materials standard F1292-99. The composite structure on the preschool playground did not have the required amount of resilient surfacing. There was about 5inches of mulch in the equipment's fall zones where 9 inches was required.
