Inspector Notes

A monitoring inspection was conducted on 5/5/2023 with the licensing inspectors on site from approximately 10:30 am to 1:45 pm. The center director and center staff were available throughout the inspection to answer any questions. There were 75 children present with 17 staff supervising. The inspectors reviewed compliance in the areas of administration, physical plant,
staffing and supervision, programming, medication, special care and emergencies and nutrition. The center is equipped with toys, supplies and items were available to children. Medication is administered when required and medication and medication authorizations were reviewed.
During the inspection, the children and staff were observed participating in a variety of activities to include outdoor play. Interviews were conducted with staff and areas of the facility were inspected to include all classrooms, bathrooms and the playground areas.
A total of 8 child records and8 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-550-P
Based on record review, the center did not ensure that the written record of children's serious and minor injuries contained the required information.
Evidence:
1. One of the five injury records reviewed did not contain two signatures.
2. Three of the five injury records reviewed did not contain the date the parent was notified.
3. Four of the five injury records reviewed did not contain the method of parent notification.
4. Five of the five injury records reviewed did not contain the time when parents were notified.
Plan of Correction: Will train all staff completing all incident reports.
Standard 8VAC20-780-60-A
Based on record review, the center did not ensure three of eight children's records contained all of the required information.
Evidence:
1. The record of child #4 (enrollment date: 10/12/2018) did not contain documentation of the annual parent confirmation that the required information in the child's record is up to date.
2. The record of child #6 (enrollment date: 3/21/2022) did not contain documentation of the annual parent confirmation that the required information in the child's record is up to date.
3. The record of child #7 (enrollment date: 8/22/2021) did not have documentation of all required information until 3/17/2023. The center representative stated that the center could not locate the child's record and had to create a record for the child dated 3/17/2023.
Plan of Correction: Within the next few weeks we will update every ones annual information (by May 30th) - Keep every ones file updated
