Inspector Notes
The licensing inspector conducted an unannounced inspection on Thursday, June 22, 2023 in response to a self-reported incident that the Department received on June 15, 2023 regarding special feeding needs. The inspector was on site at the center on June 22, 2023 from 3:15pm to 6:00pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.
The preponderance of the evidence gathered during the investigation validates the self-reported incident. A violation was cited regarding special feeding needs. See the violation notice for more details.

Violations
1Evidence:
1. The center's representative and staff #1 reported the following: On 06/15/2023, staff #1 fed child #1 (Age 5 months) a bottle between approximately 9:58am and 10:30am. Staff #1 did not feed child #1 for the rest of the day. The interviews also revealed that a parent picked up child #1 between approximately 4:00pm and 4:15pm.
2. The parents of child #1 instructed the center staff to feed child #1 two bottles, four hours apart.
3. Staff #1 stated that she mixed up child #1?s feeding schedule with another child?s feeding schedule.
