Inspector Notes
A Subsidy Health and Safety inspection was conducted on 11/14/2023 to review supplemental health and safety requirements. There were 96 children present with 18 staff supervising.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on a review of nine staff records and interview, the vendor did not ensure that three staff annually completed the department's health and safety update course as required.
Evidence:
1. The record of staff #6 contained a course certificate dated 9/20/2022. The record of staff #8 contained a certificate dated 11/12/2022 and the record of staff #9 contained a certificate dated 8/14/2022.
2. The vendor acknowledged that the courses had not been completed annually as required.

Plan of Correction: Going forward all staff will complete the health and safety training each year. All staff needing the training updated will have it completed by 12/31/2023.
