Inspector Notes

An unannounced monitoring inspection was initiated on 11/8/2024 and completed on 11/15/2024 with local agencies, in response to a self-report received by the licensing office on 10/25/2024 relating to behavior guidance and forbidden action. The inspector reviewed 1 child record and 1 staff record on-site. This inspection included document review, interviews and observations.

Though not a part of the original self report, a violation that was not reported was found related to background checks and is documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/22/2024. A POC submitted after this date will not appear on the public website.
