During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 3/12/2025 and completed on 3/13/2025. The on-site inspection began at 12:24 pm and ended at 3:35 pm. The inspector reviewed compliance in the areas listed above. There were 94 children present and 19 staff. The inspector reviewed 9 children?s records on site and 9 staff records electronically on 3/13/2025. This inspection included document review, a tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/21/2025.
A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

The plastic mulch border located in the preschool playground was cracked/broken in 5 places. The chain link gate in the two year old playground had unraveled metal wire pieces along the bottom that exposed sharp edges.
Plan of Correction: Put in work order at time of visit. Awaiting plan from property.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following: 1. Date and time of injury; 2. Name of injured child; 3. Type and circumstance of the injury;
4. Staff present and treatment; 5. Date and time when parents were notified; 6. Any future action to prevent recurrence of the injury; 7. Staff and parent signatures or two staff signatures; and 8. Documentation on how parent was notified.

Child #10 had two incomplete reports that were missing the second signature and documentation on how the parent was notified.
Plan of Correction: Staff was given impromptu meeting and reshown the proper way to fill out reports. AD will check and sign off then give to CD for last phase.
