Inspector Notes

An unannounced inspection was initiated on 6/10/2025 and completed on 6/25/2025 in response to a complaint received by the licensing office on 6/04/2025 relating to allegations in the areas of behavior guidance and forbidden action. The inspector reviewed 2 children?s records and 1 staff record electronically on 6/24/2025 and 6/25/2025. This inspection included document review, interviews, and observations.

The preponderance of evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
