Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 3/23/2021 and concluded on 3/24/2021. The director was contacted by telephone to initiate the inspection. There were 32 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 6 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard 22VAC40-191-60-C-2
Based on record review and interview, the center did not obtain a Virginia search of child abuse and neglect results for a staff member by the end of the 30th day of employment.

Evidence:
1. Staff #1,(Date of hire 08/2020), did not have documentation of a central registry finding available for review.
Plan of Correction: We now have the documentation needed after a second attempt with Central Registry.
Standard 63.2(17)-1721.1-A
Based on review of Board Officers records, the center did not update Sworn Disclosure Statements for each agent every 5 years.

Evidence:
1. The records of Agent#1 and Agent#4 contain Sworn Disclosure Statements dated 9/01/2015. Both documents were completed more than 5 years ago.
Plan of Correction: Agen#1 and #4 have both signed and updated.
