Inspector Notes

An unannounced renewal inspection was conducted today from 10:45am-1:20pm. There were 36 children (infants-5yrs.) directly supervised by 7 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play and playing on the playground. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized.

Based on record review, the child day center did not obtain abuse and neglect results every five years as required.

Evidence:
1.The record for Staff #1 contained a Central Registry Check that was last updated on 7/18/2017.
2.The record for Staff #5 contained a Central Registry Check that was last updated on 12/21/2012.
Plan of Correction: The proper paperwork was filed for both staff members.

Based on record review, the center did not obtain, out-of-state criminal record reports and out-of-state abuse and neglect reports for a staff member that has resided outside the state of Virginia within the last 5 years.

Evidence:
Staff #2's (DOH: 9/1/2022) record did not contain criminal history reports prior to their first day of employment and abuse and neglect results within 45 days of employment for states where the staff member has resided within the past 5 years.
Plan of Correction: The application was resent to the missing state.
