During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/31/2026, as a part of the licensure period. The on-site inspection began at 9:25am and ended at 11:10am. The inspector reviewed compliance in the areas listed above. There were 41 children present and 6 staff. The inspector reviewed 4 children?s records and 2 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
The center is required to have employees undergo a background check every five years.

The most recent central registry check for Staff #1 is overdue.
Plan of Correction: Administrator to research location of background request/result or submit request for new one today.
Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

The center did not request a search of the child abuse and neglect registry from one state for Staff #1, who lives in another state, as stated on Sworn Statement 2023.

Standard 8VAC20-780-160-C
Staff members shall obtain and submit the results of a follow-up tuberculosis screening, at least every two years from the date of the first initial screening or testing.

The TB screening for Staff#1 has not been submitted and is overdue by 7 months.
Plan of Correction: Administrator will have staff be screened by agency nurse, today.

Staff records shall contain name, address, and telephone number of a person to be notified in an emergency.

Staff #2's record did not contain address of emergency contact person.
