Inspector Notes

A monitoring inspection was conducted on 09/27/2023. There were a total of 46 children with 9 staff members present. Children were observed playing outside, cleaning up small group centers and eating lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on a review of staff records, the CDC did not ensure required background checks were conducted from any state in which the individual has resided in the preceding five years.
Evidence:
Staff #1 (date of hire 9/6/2023) did not have Colorado CRS documentation on site during the inspection.
Plan of Correction: Did have sex offender search done and Colorado criminal history check completed. Called Colorado department of division of early care and learning to get the correct form completed and submitted .
