Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a review of video footage.

A monitoring inspection was initiated on 5/17/21 and concluded on 7/16/21. A self-reported incident was received by the department regarding allegations in the area of supervision. The director was contacted by telephone to conduct the investigation. Interviews with staff were conducted by phone. Video footage, as well as documentation related to the incident, was also reviewed. The evidence gathered during the investigation did not support the self-report of non-compliance with standards or law.
