Inspector Notes

A complaint inspection was conducted on August 18, 2023, from 11:00am to 2:15pm in response to a complaint that was received by the department on July 31, 2023, regarding allegations in the care and protection of the children at the center. Staff were interviewed, documentation and records were reviewed, classroom observations were completed, and a tour of the facility was conducted. Information gathered during the inspection determined non-compliance with applicable standards and the complaint is determined to be Valid. A violation was documented on the violation notice issued to the program. The violation was reviewed and discussed with the site Director at the exit interview.

Based on observation and interview, it was determined that the center did not ensure that when staff are supervising children, they shall always ensure their care, protection, and guidance.
Evidence:
1. On July 28, 2023, Child #1 was instructed by Staff #2 to clean up feces off the daycare floor.
2. District Manager, Staff #3, confirmed watching the video footage from the morning of July 28, 2023, in which Child #1 was observed to clean up feces off the daycare floor.
3. Staff #2 has been terminated as Assistant Director as it is noted that they were observed being unprofessional and inappropriate by allowing a child to clean up another child?s feces.
4. Staff #4 confirmed seeing the feces on the floor and that Staff #2 confirmed to them that they had Child #1 clean up feces off the floor.
Plan of Correction: The center responded with the following: As a result of this incident, Staff #2 was terminated.The center will ensure to re-train all staff regarding the care, protection, and guidance of children at the center.
