During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on January 16, 2025. The on-site inspection began at 11am and ended at 1:40pm. The inspector reviewed compliance in the areas listed above. There were 42 children present and 11 staff. The inspector reviewed 6 children?s records and 4 staff records on-site on January 16, 2025. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-500-A
Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks and shall be washed with soap and running water after toileting. Infant staff was observed diapering an infant child and taken directly to eat lunch without washing hands.
Plan of Correction: The center responded with the following: During the time of the inspection, the staff member was prompted to wash the child?s hands. At the next unit meeting, staff will be retrained on handwashing.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the date and time when parents were notified and documentation of how parents was notified. In reviewing the November injury reports, there were 4 reports that did not have the date and time and documentation of how and when parents were notified.
Plan of Correction: The center responded with the following: At the next unit meeting, staff will be re-trained on documentation of injury reports. Staff will be able to also practice writing injury reports during this training. Director will review each injury report to make sure that all required documentation is in the report.
Standard 8VAC20-780-570-A
When a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. Three infants were observed eating lunch in a high chair and were not securely fastened with a protective belt.
Plan of Correction: The center responded with the following: The center will ensure that protective belt straps will be fastened securely when in a high chair and repair any straps that are broken.
