During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on September 17, 2025, and completed on September 26, 2025, in response to a self-report received by the licensing office on September 17, 2025, relating to child supervision and transportation. The inspector reviewed children?s records and three staff records on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 14, 2025. A POC submitted after this date will not appear on the public website.

Violations
31. Per the center?s supervision policy and procedures, children shall be supervised at all times and a 5 year old child was left alone and unsupervised in the school?s transportation vehicle.
2. Per the center?s transportation procedures, staff were to walk to the rear of the bus, looking for the children in under and around each seat. Staff did not check the rear of the bus which resulted in a child being left unsupervised in the school?s transportation vehicle for approximately 16 minutes.
Center director will conduct a retraining all current staff including management team on LCG's Transportation procedures, Face to Name Procedures & Supervision Policy during the October staff meeting.
