Inspector Notes

An unannounced monitoring inspection was conducted on 9/21/2022. There were 20 children, ages 3 years-4 years and 4 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 2 children?s records and 5 staff records were reviewed. The children were observed during circle time and active time on the playground. The inspector arrived for the inspection at 9:45am and departed the center at 10:57am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on review of five staff?s records, the center failed to ensure that an employee was not employed until the center had the person?s out of state criminal history results.

Evidence: Staff #4 was hired on 11/19/2018 and on the day of the inspection did not have the required out of state criminal history check completed.
Plan of Correction: Provider will obtain out of state criminal history check for staff.
