Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/28/2024 and completed on 10/28/2024, as a part of the licensure period. The on-site inspection began at 11:56AM and ended at 12:42PM. The inspector reviewed compliance in the areas listed above. There were 24 children present and 6 staff. The inspector reviewed 3 children?s records on site on 10/28/2024 and 4 staff records on 10/25/2024. Medication was reviewed on site 10/28/2024.

This inspection included - document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, and observations (this would include observations of photos, recordings, etc.).

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

A written agreement between the parent and the center shall be in each child?s record by the first day of the child?s attendance. The record for Child #3, in care for 2 months, did not contain a written agreement between the parent and the center.
Plan of Correction: Provider obtained written agreement from parent.
