Inspector Notes

A renewal inspection was initiated on 6/3/2024 at approximately 10:30am and concluded on 6/3/2024 at approximately 12:40pm. There were a total of 43 children with 14 staff members present. Children were observed participating in activity stations, making play dough, preparing to nap, engaged in group activities and outdoor play time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 8 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of center's board of directors documents, it was determined that out of state criminal history record checks were not completed.

Evidence: There was no out of state criminal history record check for Individual #4.
Plan of Correction: Center will obtain the out of state background check for the individual.

Based on review of children's information, it was determined that the center did not obtain work phone numbers of each parent who has custody.

Evidence: There were missing work phone numbers for Child #1, Child #2, and Child #3.
Plan of Correction: Center will obtain work phone numbers for the parents.
