During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 2/3/2025 and completed on 2/7/2025 in response to a self-report received by the licensing office on 1/31/2025 relating to supervision of children. This inspection included document review, tour of the facility, interviews, and observations. The inspector reviewed two children?s records and two staff records on site. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to background checks and are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
The center is required to have employees undergo a background check every five years. The most recent central registry check for staff #1 was overdue by six weeks.
Plan of Correction: The central registry request has been submitted and the results will be placed in the staff's file.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection and guidance. On 1/30/2025, no staff were present to offer immediate intervention and assistance when two children (both three years old) were left alone outside the main entrance of the center for approximately three to five minutes, with no barrier to protect them from walking 29 feet towards the adjacent parking lot and 72 feet towards the street (with a 25 mile-per-hour speed limit). When Staff #1 and Staff #2 realized the children were missing, they returned to the front door and found both children standing on the doorstep of the building.
Plan of Correction: Both staff member have received written reprimands in their files, along with re-training on Face-to-Name procedures, Transitions and Ratios. Furthermore the entirety of staff will receive re-training on Face-to-Name procedures at our next staff meeting in late February.
Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff. On 1/30/2025, Staff #1 and #2 were transitioning 13 children from the outside playground back inside to their classroom, and left two children (both three years old) unsupervised outside the main entrance of the center, for approximately three to five minutes.
Plan of Correction: Both staff member have received written reprimands in their files, along with re-training on Face-to-Name procedures, Transitions and Ratios. Furthermore the entirety of staff will receive re-training on Face-to-Name procedures at our next staff meeting in late February.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards to identify where children are at all times.

On 1/30/2025, Staff #1 and Staff #2 failed to follow the center's policy on performing a face-to-name check after the class transitioned from outdoor play, and as a result two children were left outside at the top of the stairs while the rest of the class went inside from the playground.
