During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be
included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 6/11/2025, as a part of the center's three-month probationary status. The Department issued a Special Order on May 5, 2025, to place the licensee on probation and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations as a result of high-risk violations. This inspection was also in response to a self-report received by the licensing office on 6/4/2025 relating to supervision. The on-site inspection began at 10:25am and ended at 11:50am. This inspection included tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/1/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation

When staff are supervising children, they shall always ensure their care, protection and guidance. On 6/3/2025, Staff #1 and Staff #2 staff were performing a face-to-name check while transitioning a group of children from a classroom on the third floor to another classroom on the second floor, when Child #1 ran ahead of the group and went down the stairs to the classroom on the second floor. The staff did not go after Child #1 until the group had finished the face-to-name check at the top of the stairs. Neither staff would have been able to offer assistance to Child #1 during this time away from the group.
Plan of Correction: Staff have been retrained on proper name to face procedures. Teachers are working with students on safety and staying with teachers until reaching their destination.

A kickstand has been purchased and will be installed soon.

Repeat Violation

Children under 10 years of age shall always be within actual sight and sound supervision of staff. On 6/3/2025, Child #1 (3 years old) went out of sight and sound of Staff #1 and Staff #2 who were supervising the group for less than one minute when they ran ahead of the group into an unoccupied classroom on the floor below.
Plan of Correction: Staff have been retrained on proper name to face procedures. Teachers are working with students on safety and staying with teachers until reaching their destination.
