During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 8/7/2025, as a part of the center's three-month probationary status. The Department issued a Special Order on May 5, 2025, to place the licensee on probation and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety as a result of high-risk violations. An unannounced monitoring inspection was also conducted. The inspector reviewed compliance in the areas listed above. There were 50 children present and 11 staff. The inspector reviewed six children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #4, #5, and #6, who were all in attendance on the day of inspection.
Plan of Correction: A physical exam will be obtained for all children attending going forward.
Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis (TB) screening. Documentation of a negative TB screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. An independent contractor, working with children during the inspection had no documentation of a negative TB screening.
Plan of Correction: A negative TB screening and/or test will be obtained for all staff and contractors.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Chipped paint and pealing wallpaper was found in the 3-years to 4-years classroom. Pealing paint was found in the stairway of the center. A wooden structure on the playground had significant wear and was a risk of splinters. The shed on the playground had 2 broken window panes.
Plan of Correction: Licensee is working with facilities to get these items corrected.
Standard 8VAC20-780-280-B
Hazardous substances must be in a locked place that prevents access by children. In the 18-month to 2-year old classroom, the 3-year to 4-year old classroom, and the 4-year to 5-year old classroom, bleach was found in an unlocked area.
Plan of Correction: All items will be kept in locked cabinets moving forward.

In areas used by preschool age or younger children, electrical outlets shall have protective covers. There was an uncovered outlet in the 3-years to 4-years classroom.
Plan of Correction: All electrical outlets will be kept covered with outlet covers.

Repeat Violation

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. Two children in attendance were outside the age range of the license.
Plan of Correction: Licensee will adhere to the age range of the license.
Standard 8VAC20-780-510-L
Medication, except for those prescriptions designated otherwise by written physician's order, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. Medication was found in an unlocked cabinet, out of reach of children, in the 4-year to 5-year classroom.
Plan of Correction: Lock box will be locked with key at all times.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The records for Child #4, #5, and #6, all in attendance on the day of inspection, were missing the following required components: name and phone number of child's physician, documentation of viewing proof of the child's identity and age, and name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached
Plan of Correction: Licensee had parents complete updated forms.
Standard 8VAC20-780-90-A
A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent and include:

1. An authorization for emergency medical care should an emergency occur when the parent cannot be located immediately unless the parent states in writing an objection to the provision of such care on religious or other grounds;
2. A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; and
3. A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.

The records for children #4, #5, and #6, present on the day of inspection, did not have this written agreement.

Standard 8VAC20-820-120-E-6
A copy of any special order issued by the department shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations. A copy of the special order was not posted on the premises.
Plan of Correction: Special order was posted in the correct spot as soon as possible.
