Inspector Notes
An unannounced monitoring inspection was initiated on and completed on 8/19/2025 in response to a self-report received by the licensing office on 8/14/2025 relating to supervision of children. The Department issued a Special Order on May 5, 2025, to place the licensee on probation and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety as a result of high-risk violations. This inspection included document review, tour of the facility, interviews, and observations. The inspector reviewed two staff records on site. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to background checks and are documented on the violation notice issued to the program.

Systemic Deficiency
When staff are supervising children, they shall always ensure their care, protection and
guidance. On 8/14/2025, no staff were present to offer immediate intervention and assistance
when two children (both three years old) were left alone in the bottom of the stairway for approximately one to two minutes. When Staff #2 heard one of the children crying, they located the children and united them with the class.
Industrial door wedge/door stopper was purchased and is available for staff to use to hold open door.
Systemic Deficiency
Children under 10 years of age shall always be within actual sight and sound supervision of
staff. On 8/14/2025, Staff #1 was transitioning ten children from their classroom to the music room on the floor below, and left two children (both three years old) unsupervised in the stairway for approximately one to two minutes.
