Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 12/17/2025, as a part of the center's nine-month probationary status. The Department issued a Special Order on October 27 2025, to place the licensee on probation and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety as a result of high-risk violations. The inspector reviewed compliance in the areas listed above. This inspection included tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
