Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on 08/11/2021 and concluded on 08/23/2021. The director/provider or in-charge person was contacted by telephone to initiate the inspection. There were 64 children present and 11 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center did not obtain a copy of the results of a criminal history record information check and a sex offender registry check, from any state in which the individual has resided in the preceding five years.

Evidence:
1. Staff #2 has a hire date of 4/5/2021. Staff #2 indicated on her sworn statement or affirmation that she has resided in the state of Pennsylvania within the past five years. The results of a criminal history record check were not available for staff #2 from the state of Pennsylvania .
2. The center director confirmed that the required out-of-state background checks for staff #2 had not been obtained.
Plan of Correction: The director stated the request for criminal record check was sent to Pennsylvania but results were not received. In the future staff will not be hired until out of state criminal record results are verified.
