An unannounced complaint investigation was conducted on 7/28/2022 regarding special services.
Interview was conducted, observation were conducted and records were reviewed. The preponderance of evidence support the allegations in regards to special servcies and the complaint has been deemed VALID.

Based on staff interview and review of menu, the licensee did not ensure center follow the most age approriate nutritional requirements of the Child and Ault food Program for USDA.
Evidence: On July 28, 2022 the menu posted in the facility shows for breakfast there are only 2 food components provided for breakfast. USDA guidelines state the center must provide 3 components for breakfast. Staff #1 confirmed the menu does state breakfast but it is actually a snack provided in the morning.
