Centers shall obtain a completed national criminal fingerprint background check prior to the employee's first day of employment. 1. The record of staff #1, employed for one month, contained results of a national criminal fingerprint background check dated four days after employment. 2. The record of staff #2, employed for three weeks, contained results of a national criminal fingerprint background check dated one day after employment.
Plan of Correction: Per the director: We mixed up the CRS and fingerprint requirements. It has been noted of the correct time frames.

Each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. The record of staff #2, employed for 24 days, contained a tuberculosis screening dated 20 months prior to employment.
Plan of Correction: Per the director: I will update staff records.

Staff shall obtain and submit the results of a follow-up tuberculosis screening at least every two years from the date of the initial screening or testing. The record of staff #4 contained documentation of a tuberculosis screening that expired five months ago.
Plan of Correction: Per the director: An appointment has been scheduled to obtain an updated TB.

The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. 1. Staff #3, employed for one years and three months, did not complete the Virginia Department of Education- sponsored orientation. 2. Staff # 5, employed for one year and one month, did not complete the Virginia Department of Education- sponsored orientation.

Standard 8VAC20-780-245-A
Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. 1. The record of staff #3 contained documentation of seven hours of annual training for the period of 07/29/2024 through 07/28/2025 when 16 hours was required. 2. The record of staff #4 contained documentation of seven hours of annual training for the period of 10/26/2024 through 10/25/2025 when 16 hours was required. 3. The record of staff #5 contained documentation of three hours of annual training for the period of 09/23/2024 through 09/22/2025 when 16 hours was required.

The center shall obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. The record of child #6, with a diagnosed food allergy, did not contain an allergy care plan.
