Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A monitoring inspection was initiated on 8/27/2021 and concluded on 9/3/2021. The director was contacted by telephone and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center has not obtained a subsequent tuberculosis (TB) screening every 2 years for each staff member.
Evidence: Staff #1's most recent TB screening is dated 7/22/2019. The center Director confirmed that the TB screening was expired.
Plan of Correction: They have an appointment for a follow-up TB test on 9/7/2021.
