Violations
3Evidence: The children's bathroom in the Discovery Preschool classroom did not contain toilet paper.
Evidence: Child #4's emergency medication was stored in a backpack in the child's classroom. The medication was inside of a safety bag, however the lock on the bag was missing.
Evidence: Child #3's physical exam documents a diagnosed food allergy. The program director confirmed that they have not obtained an allergy care plan for the child.
