Inspector Notes
Today an unannounced Monitoring Inspection took place between the hours of 9:10am and 11:45am. There were 8 classrooms observed with a total of 87 children with 15 staff within the supervision guidelines. The children were observed playing on the playground, participating in circle time, singing songs, working on art drawings and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log and emergency procedures were observed during this inspection. A sample of 5 medications and authorizations were reviewed. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations
9Evidence: Staff #6's fingerprint background check is dated 5/21/2018. The program director confirmed that the staff person had not yet obtained a repeat fingerprint background check.
Evidence:
1. Staff #2's (date of hire: 2/6/2023) sworn disclosure statement confirmed that they have lived in the state of Arizona within the last five years. An Arizona central registry background check has not been obtained.
2. Staff #7's (date of hire: 2/7/2023) sworn disclosure statement confirmed that they have lived in the states of Colorado and Georgia within the last five years. A Colorado and Georgia central registry background check has not yet been obtained.
Evidence: The light in the boys bathroom of the transitional kindergarten classroom does not work. The bathroom is completely dark.
Evidence: Child #7 (11 months) was not laid on their back to rest. The caregiver laid the child on their tummy and began patting the child's back to put them to sleep.
Evidence: Emergency medication for Child #9 was observed in the transitional kindergarten classroom. The medication expired 5/10/2023. The program director stated that the medication should have been returned to the family to avoid unintentionally administering expired medication.
Evidence: A staff member in the toddler classroom was observed changing a child's diaper. After the diaper change was complete, the child was placed on the floor and returned to participate in circle time. The teacher logged the diaper change on the classroom tablet and joined the group for circle time. The child's hands were not washing and the teacher did not wash their hands after the diaper change.
Evidence: Emergency medication for Child #8 was located in a backpack hanging in the pre-k classroom. The medication was in a bag that was not locked.
Evidence: Four bottles labeled for Child #6 were observed in the infant room refrigerator. The bottles did not contain today's date.
Evidence: Staff #7's record did not contain the name, address and phone number of an individual to be contacted in case of an emergency.
