An unannounced monitoring inspection was initiated on 06/16/2025 and completed on 06/16/2025 (from 8:35AM to 9:30AM), in response to a self-report received by the licensing office on 6/9/2025, and an additional report received by a local agency on 6/10/2025. The inspection was completed relating to Staffing & Supervision and Programs. The inspector reviewed 2 children?s records and 1 staff record on-site on 6/16/2025. This inspection included document review and interviews.
