During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 12/03/2024 and completed on 12/11/2024. The on-site inspection began at 10:00 AM and ended at 12:15 PM. The inspector reviewed compliance in the areas listed above. There were 45 children present and 6 staff members. The inspector reviewed 5 children?s records, and 5 staff members on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/27/2024. A POC submitted after this date will not appear on the public website.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Standard 8VAC20-780-160-B
The results of a negative tuberculin skin test (TST). The documentation shall include the date the test was given and results of the test and be signed by a physician, physician's designee or an official of the local health department.

Staff # 3 and 4 TB screenings were not signed by the physician, physician's designee or an official of the local health department.

Inside areas occupied by children shall be maintained no lower than 68 degrees F.

The temperature in two occupied classrooms (toddlers and preschoolers) was 62.5 to 63 degrees.
Plan of Correction: A heating and air company was called and scheduled to come out to fix the problem with no heat.
