During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 06/02/2025 and completed on 06/08/2025. The on-site inspection began at 10:20 AM and ended at 12:00 PM. The inspector reviewed compliance in the areas listed above. There were 45 children present and 7 staff. The inspector reviewed 5 children?s records onsite, 4 staff virtually. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 06/13/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

The wood on the large climber has deteriorated and has splintering wood.
Plan of Correction: School will replace play structure over summer while students are away.
Standard 8VAC20-780-280-B
Hazardous substances must be in a locked place that prevents access by children.

In the two-three-year-old classroom, the cleaning solution and sanitizing solution were in an unlocked cabinet, out of reach of children.
Plan of Correction: Staff will be reminded to lock all chemicals in locked cabinets.
Standard 8VAC20-780-330-B
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall.

The fall zones on the swings did not measure double the height of the swings.
Plan of Correction: Small fence liner in front of swings will be removed to ensure fall zone compliance.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

At approximately 10:20 a.m. on 06/02/2025, there were 12 children present in the two-three-year-old room but only 8 were marked present on the attendance record.
Plan of Correction: Staff will be reminded of procedures for daily attendance. ensuring all students are signed in and out.
